Case Management Services

Our case management services provide individualized support and guidance for children and adults with developmental disabilities. We work collaboratively with individuals and their families to identify strengths, needs, and personal goals while promoting choice, independence, and self-advocacy. Through coordination of community resources, service planning, and ongoing monitoring, we help ensure access to appropriate supports such as medical care, educational services, therapeutic interventions, and community-based programs.

With over 15 years of experience in human services, developmental disabilities support, and systems-based service coordination, our leadership brings extensive knowledge in case management operations, regulatory compliance, and program oversight. Our background in upper management and service delivery systems ensures that processes are structured, organized, and aligned with state and federal standards while remaining person-centered at every level. 

 Our goal is to empower each person to live as independently and meaningfully as possible within their home and community.
